Family Podiatry of Belmont Payment Policies

Payment for our services, including a co-payment, is expected at the time of your visit.

We accept all major insurance plans, including Medicare, with the appropriate referrals, ID numbers and cards, and paperwork. Our staff will be glad to file the insurance forms for you. Be sure you have your insurance cards with you for your first visit, or if there have been changes to your insurance company or coverage. We will need to make a copy of these cards for our records.

If your insurance plan requires a co-payment, please be prepared to pay this when you check in. If you have any questions regarding your insurance coverage, our staff will be happy to help you.

Most insurance carriers will pay for services considered "reasonable and necessary", and many plans will cover specific diagnostic/screening tests, vaccines, and preventive care services. Please be aware that some services and procedures have limitations associated with them as well as deductibles. Again, if you have questions regarding any of the above, please ask either the office staff or Dr. Alper himself. We are happy to help you!

If your insurance carrier denies payment for any reason, including non-covered services, deductible limits not met, or terminated insurance coverage, you do remain responsible for any unpaid balance on your account.

We accept as payment cash and checks, as well as credit cards.
